	The title of our book, name of author or editor, the page number(s) of the material to be reproduced, and (if applicable) figure or illustration numbers; for magazines we need the issue and year number, page number(s) of the material to be reproduced, name of author/illustrator
	Number of words and/or pages to be reproduced
	Title of your book, your author or editor, the name of your publisher, expected month/year of publication, distribution (e.g. U.S only, world), estimated number of copies to be printed, and estimated list price
